Waiver Form

Please read each section carefully and then sign and date at the bottom. This waiver is required for all explorers before payment.

Consent & Releases

1. Program Participation

I understand that my child is voluntarily participating in STEM Wonder Lab activities, which may include hands-on experiments, movement, and interactive learning experiences. I acknowledge that while all reasonable safety precautions are taken, participation involves inherent risks.

2. Health & Safety

I agree to inform STEM Wonder Lab of any medical conditions, allergies, or special needs that may impact my child’s participation.

3. Behavior Expectations

STEM Wonder Lab is a structured, safe, and positive environment. I understand and agree that students are expected to follow directions and participate safely. Aggressive, unsafe, or extreme behaviors will not be tolerated. If a student demonstrates unsafe or extreme behavior, continued participation may be revoked at the discretion of STEM Wonder Lab.

4. Pick-Up Policy

I agree to pick up my child on time at the scheduled end of the program. A grace period of 10 minutes is allowed. Late pickups may result in a late fee. I understand that supervision is not guaranteed beyond the scheduled program time.

5. Attendance & Refund Policy

Missed sessions are non-refundable unless otherwise stated. STEM Wonder Lab reserves the right to cancel or reschedule sessions as needed.

6. Liability Waiver

I release and hold harmless STEM Wonder Lab, its owner, and staff from any and all liability for injuries, damages, or losses that may occur during participation, except in cases of gross negligence.
